Time to Care for You
GCHD’s primary care providers serve as the go-to “medical home” for you and your family. Your primary care physician, physician assistant, or nurse practitioner provides a consistent home base to connect all your care through one centralized source that provides direction and helps you navigate each new age and stage of wellness.
At GCHD’s Pomeroy Medical Clinic, your primary care team works to build a relationship with you and your family over time, including your family medical history and any personal health preferences. Research shows that patients who establish a relationship with a primary care provider tend to be healthier overall. Additionally, patients will benefit from:
1. Continuity
From one appointment to the next, you can feel confident in your primary care provider’s growing understanding of your health. Having a go-to primary care provider create stability and continuity when it comes to your care. This is especially critical for people with chronic conditions, such as arthritis, diabetes, or heart disease.
2. Communication
As you and your primary care provider get to know each other better, you develop a deeper level of trust and better communication, making it easier to share more sensitive information or ask more difficult questions.
3. Maintaining your health
As your primary care provider develops a better idea of what is normal for you, it becomes easier to recognize potential problems or to recommend specific types of preventive care. For example, if heart disease runs in your family, your primary care provider will likely recommend certain lifestyle changes, while monitoring your vitals more regularly.
4. Early detection
Recurring checkups with the same physician make it more likely to catch health issues early on – when they are easiest to treat.
Ideally, you should see your primary care provider once or twice a year for wellness visits, health screenings, and any necessary acute care when you are sick or injured.
