Medical Records Request

Patients who have received care at Garfield County Hospital District may request copies of their medical record/health information. Please make sure to include the following information to help ensure quick processing of your request:
  • Name (or patient’s name)
  • Date of birth
  • Social security number
  • Date of service and treatment at Garfield County Hospital
  • Your address or the address to which you are sending your records
  • Your telephone number
  • Your signature

Requesting Medical Records of a Deceased Patient
Please contact the Garfield County Hospital Business Office at 509-843-1591. You may need to provide additional documents in order to obtain a copy of the records.
In order to get a copy of your medical records from Garfield County Hospital, you must submit a signed and dated Information Release Form.
