TRANSITIONAL CARE

Elderly care and support

When it's not time to go home, but you're well enough to leave acute care.

Garfield County Hospital District offers transitional care services for patients that are ready to leave traditional acute care in the hospital but are not yet ready to return home or to their previous living situation. These patients still require additional skilled medical care, nursing care or rehabilitation services.
 
Garfield County Hospital District’s transitional care team is trained and equipped to provide care for patients with non-acute, but still complex needs, including:

 

  • Post-surgical care, including cardiac, neuro, orthopedic and abdominal

  • Respiratory care

  • Wound care for wound healing and prevention

  • Intravenous (IV) antibiotics and therapy to treat a variety of infections and conditions

 
Additionally, our transition care team works in collaboration with other services at Garfield County Hospital District to provide rehabilitation care during a transitional care stay, such as:

 

  • Physical therapy

  • Occupational therapy

  • Speech therapy

  • Rehabilitative therapy supportive services
